Any senior with a 3.50 or higher cumulative grade-point average or Schreyer's Honor Scholar may be admitted to 500 or 800-level courses with only the consent of the instructor. (Advisor signature only needed when GPA is lower than 3.5.)
Any senior with a 3.00 to 3.49 cumulative grade-point average may be admitted to 500 or 800-level courses by obtaining all signatures listed below.
Any Integrated Undergraduate-Graduate (IUG) student should use this form to enroll in graduate level courses that are to appear on the student's undergraduate record (instructor and adviser signatures not required).
Please submit all forms to Graduate Enrollment Services for approval by the Regulare Add Deadline. Forms submitted after the deadline will be denied. Upon GES approval, this form will be forwarded to the Registrar's Office.

Candidates for graduation during a particular semester may request an official letter that verifies their status with respect to meeting degree requirements and their expected graduation date. Potential employers, licensure boards, embassies and other agencies may require this type of verification, including petitions for H-1B visas.
