Application Fee Form

You have chosen to pay the application fee by personal check or money order.  This form must accompany the application fee payment.  Your application will not be processed until the fee is received.  The application fee is nonrefundable.

Instructions:

  1. Download and print the Application Fee Form (PDF)
  2. Complete hard copy of form
  3. Return Application Fee Form and Payment to:

    Graduate Enrollment Services
    The Pennsylvania State University
    114 Kern Graduate Building
    University Park, PA 16802

Payment Method:

  • Enclose either a personal check or money order with the completed Application Fee Form. Do not send cash.
  • International money order or check must be drawn on a U.S. bank. 
  • Make checks payable to:  The Pennsylvania State University
  • The canceled check serves as your receipt.
